Privacy Policy for Form

1. Introduction

Form (“Form”, “we”, “our”, or “us”) is a workout logging and coaching app published by MaveriKode. This Privacy Policy explains what information we collect when you use Form on Android or the web, how we use it, and what choices you have.

By creating an account or using Form, you agree to this Privacy Policy.

2. Information We Collect

Account information

When you sign up, we collect:

• Username
• Email address
• Password (stored securely; we never store it in plain text)
• Display name
• Body metrics you provide: height, weight, age, and sex (used for diet and training features)

Workout and fitness data

When you use the app, we collect and store:

• Workouts, exercises, sets, reps, weights, duration, and distance
• Personal records (PRs) and training history
• Assigned training programs and diet plans
• Workout notes and comments (if you add them)

Some of this data is stored locally on your device first (for offline use) and synced to our servers when you have an internet connection.

AI features (Spotter)

If you use Spotter (AI program generation, diet plans, or chat), we collect:

• Your intake answers (goals, experience, equipment, injuries, food preferences, allergies, etc.)
• Chat messages you send to Spotter
• Context from your training history and assigned plans, used to answer your questions

Spotter chat messages are not shown to human coaches. Coaches may receive summarized insights (such as topic counts or pain-related keywords), not full chat transcripts.

Coach and gym features

If you belong to a gym with a human coach:

• Messages you send through the Inbox are visible to your coach
• Your coach or gym owner may view your assigned programs and member activity relevant to coaching

Payment information

If you subscribe to a paid plan, payment is processed by Paymob. We do not store your full card number. We receive confirmation of subscription status, plan type, and billing events from Paymob.

Technical data

We may automatically collect:

• Device type and operating system
• App version
• IP address and general connection data
• Error logs needed to keep the service running

We do not use third-party advertising trackers in Form.

3. How We Use Your Information

We use your information to:

• Create and manage your account
• Log workouts and show your training history
• Generate and assign training and diet plans (including AI-generated plans)
• Provide Spotter chat assistance
• Enable communication between members and coaches
• Process subscriptions and manage billing
• Improve app reliability, security, and performance
• Enforce usage limits (such as daily AI generation limits)

4. AI and Third-Party Services

Form uses third-party services to operate:

These providers process data only as needed to deliver the service. We do not sell your personal data to advertisers.

Important: Spotter provides general fitness and nutrition guidance based on your inputs. It is not medical advice. Always consult a doctor or qualified professional before starting a new program, especially if you have injuries or health conditions.

5. Data Storage and Security

• Account and synced workout data is stored on our servers.
• Active workout sessions may be stored locally on your device so the app works offline in the gym.
• Passwords are hashed. Communications with our servers use standard security measures.
• No method of transmission or storage is 100% secure, but we take reasonable steps to protect your data.

6. Data Sharing

We do not sell your personal information.

We may share data only:

• With your gym/coach — if you are a member of a coached gym (inbox messages, assigned plans, relevant training data)
• With service providers — hosting, AI, and payment processors, under confidentiality obligations
• When required by law — if we must comply with legal process or protect rights and safety

Platform administrators may access account and usage data for support, billing, and platform operations.

7. Your Choices and Rights

Depending on your location, you may have the right to:

• Access the personal data we hold about you
• Correct inaccurate profile information (via app settings or by contacting us)
• Request deletion of your account and associated data
• Withdraw consent where applicable

If you delete your account, we will delete or anonymize your data within a reasonable time, except where we must retain it for legal or billing purposes.

8. Data Retention

We keep your data for as long as your account is active or as needed to provide the service. Workout history may be retained to support your training records unless you request deletion.

Billing records may be kept as required by law and payment processors.

9. Children's Privacy

Form is not intended for children under 13 (or the minimum age required in your country). We do not knowingly collect personal information from children. If you believe a child has provided us data, contact us and we will delete it.

10. International Users

Your data may be processed on servers located outside your country. By using Form, you consent to this transfer, subject to applicable law.

11. Changes to This Policy

We may update this Privacy Policy from time to time. We will post the new version at the same URL and update the “Last updated” date. Continued use of Form after changes means you accept the updated policy.
